What an Ohio medical card costs now

The state fee is $0 for patients and caregivers. The only bill is the physician visit, once a year, and the state does not set its price.

The bill, line by line

An Ohio medical marijuana card costs $0 in state fees. The Division of Cannabis Control (DCC) says there is no activation or renewal cost for registration, and that covers caregivers too. What you pay for is the physician visit that produces your recommendation, and you need one every year.

What changed, and when

  • Until March 3, 2024: $50 a year for patients, $25 for caregivers.
  • From March 4, 2024: the DCC cut both fees to $0.01 and said it would remove them entirely.
  • Now: the DCC FAQ lists no activation or renewal cost.

One DCC page about renewing caregiver registrations, dated December 2023, still mentions a renewal fee. It predates the cut. Go by the current FAQ, and if the Registry ever asks you for money, call the DCC helpline at 1-833-464-6627 before paying.

Run your own numbers

Enter the evaluation price you were quoted and roughly what you spend at a dispensary each month. The calculator compares the visit against the 10% adult-use excise you would not pay as a patient.

Registration is free in Ohio and medical purchases are exempt from the 10% adult-use excise (Division of Cannabis Control FAQ; Ohio Department of Taxation); sales tax is not included, and the evaluation price is set by the provider.

A quick worked example with round numbers: on $200 of product a month, the 10% excise comes to $20, or $240 over a year. If your visit costs less than that, the card covers its cost on the excise alone. If you buy rarely, it may not, and the other differences on our medical vs. adult-use comparison become the real reason to register, or not.

The physician visit is the variable

Ohio does not regulate what physicians charge for a medical marijuana evaluation, and prices differ by practice. A few things to ask before you book:

  • Does the price include entering the recommendation in the Registry?
  • What does next year's renewal visit cost? You will need one, in person or by telemedicine.
  • Is the physician listed in the State Medical Board's CTR search? If not, the visit is wasted money.

Video visits are allowed for both the first and the yearly appointment; the online evaluation guide covers how they work in Ohio.

Need the recommendation part done?

In Ohio the physician starts your Registry file, so the visit comes first. Online evaluations are available for Ohio residents, and code DRWDY85872 takes 5% off. Registration itself is free.

Discounts: what is left

Ohio used to tie veteran and indigent status to a lower registration fee. With the fee at zero, no official source says what state benefit the status now brings. The DCC still records it: on August 31, 2026, it counted 26,385 patients with veteran status and 27,158 with indigent status. Dispensaries may choose to offer their own discounts, so ask at the counter. Keep in mind that a card cannot be activated while a status request is pending.

Taxes at the counter

Adult-use purchases carry a 10% marijuana excise plus state and local sales tax, both collected by the dispensary, according to the Ohio Department of Taxation. Medical purchases are exempt from the 10% excise. We could not confirm from an official source how sales tax applies to medical purchases, so the calculator leaves it out.

Costs people forget

Caregivers pay no state fee either. If a family member will shop for you, see how Ohio caregiver registration works. The bigger hidden cost is a lapse: if your registration expires, you have 7 calendar days to dispose of any medical cannabis, and you need a new recommendation before you can renew. The renewal timeline shows when to book so that never happens.

Questions people ask

Is there really no state fee?

Yes. The Division of Cannabis Control FAQ says there is no activation or renewal cost associated with state registration, for patients and caregivers alike.

When did Ohio drop the fee?

The DCC cut the $50 patient fee and $25 caregiver fee to $0.01 on March 4, 2024, with full removal to follow. The DCC FAQ now lists no cost at all.

Do veterans get a discount?

Not from the state, because there is no state fee left to discount. Patients can still register with veteran or indigent status, and some dispensaries choose to offer discounts.

Do I pay sales tax on medical purchases?

We could not confirm that from an official source. The DCC confirms only that medical purchases are exempt from the 10% adult-use excise. Ask the dispensary what appears on a medical receipt.
